1.1.1.- Business Visa (Single Entry)

U.S. Citizens

DESCRIPTION:  Business, professional or commercial purposes.

1. Application form (click here).

2. Application letter: must include applicant’s name, address, contact information, citizenship, passport number, occupation and the reason why he/she is applying for a visa.

3. Original Passport- must be valid for a minimum of 18 mo.
Please make sure that the passport is not full (must have 2 blank visas pages).

4. (2) passport pictures

5. Employment verification letter: printed on company’s letterhead, confirming applicant’s occupation. As well as the reasons why applicant is applying for a visa and the type of work he/she will be preforming/partaking in the Dominican Republic.

6. Guarantor letter from the company in the Dominican Republic confirming the applicant’s type of work and length of stay.

7. Application fee (non-refundable) of $200.00, money order, cashier’s check or company check made payable to Dominican Consulate in Chicago.

8. Pre-paid self-addressed envelope for passport/documents to be returned to applicant. We only accept USPS Priority Mail, USPS Express Mail, FedEx & UPS.

This visa allows a one-time entry to the Dominican Republic. Visa is valid for 60 days.
Allow a minimum 10-15 business days for processing. Incomplete applications will delay processing time.

Business Visa (Single Entry)

Non U.S. Citizens

7. Guarantor letter from the company in the Dominican Republic confirming the applicant’s type of work and length of stay.

8. FBI Criminal Background with an Apostille Seal from the Secretary of State of Washington D.C.

9. Application fee (non-refundable) of $200.00, money order, cashier’s check or company check made payable to Dominican Consulate in Chicago.

10. Pre-paid self-addressed envelope for passport/documents to be returned to applicant. We only accept USPS Priority Mail, USPS Express Mail, FedEx & UPS.

1.1.2.-Business Visa (Multiple Entries)

1. Application form (click here).

2. Application letter: must include applicant’s name, address, citizenship, passport number, occupation and the reason why he/she is applying for a visa.

3. Original Passport- must be valid for a minimum of 12 months. Must have 2 blank visas pages available.

4. Passport pictures.

5. Employment verification letter: printed on company’s letterhead, confirming applicant’s occupation and reasons why multiple entry visa is needed. Letter must state if employer will be covering any expenses.

6. If neither employer/company in Dominican Republic covering expenses of the trip, provide proof of economic solvency (3 consecutive bank states or check stubs).

7. Guarantor letter issued by the company in the Dominican Republic confirming the nature of the visit and length of stay.

8. Application fee (non-refundable) of $200.00, money order, cashier’s check or company check made payable to Dominican Consulate in Chicago.

9. Pre-paid self-addressed envelope for passport/documents to be returned to applicant. We only accept USPS Priority Mail, USPS Express Mail, FedEx & UPS. Must include (1) copy of every document submitted.

This visa allows multiple entries to the Dominican Republic, that should not exceed more than 60 consecutive days. Visa is valid for 12 months.
Allow a minimum of 15-20 business days for processing. Incomplete applications will delay processing time.

Business Visa (Multiple Entries)

Non U.S. Citizens

DESCRIPTION: Business, professional or commercial, as well as missionary trips.

2. Application letter: must include applicant’s name, address, citizenship, passport number, occupation and the reason why he/she is applying for a visa.

3. Original Passport- must be valid for a minimum of 12 months. Please make sure that the passport is not full (must have 2 blank visas pages).

5. Copy of U.S. Green Card/U.S. Visawhich must be valid for a minimum of 12 months.

6. Employment verification letter: printed on company’s letterhead, confirming applicant’s occupation and reasons why multiple entry visa is needed. Letter must state if employer will be covering any expenses.

7. If employer/company in the Dominican Republic is not covering expenses, provide proof of income (3 consecutive bank statements/ check stubs.

8. Guarantor letter from company in Dominican Republic confirming nature of visit and length of stay.

9. FBI criminal background check (report) with Apostille Seal from the Secretary of State of Washington D.C.

1.1.3.- Business Visa for Employment Purposes

1. Application form (click here), fingerprints are not needed.

2. Application letter: must include applicant’s name, address, citizenship, passport number, occupation and the reason why he/she is applying for a visa.

3. Original Passport- must be valid for a minimum of 12 months. Passport must have fullhave 2 blank visas pages available.

4. (2) Passport photos.

5. Non-US Citizens must submit copy of immigration status (U.S. Green Card/U.S. Visaetc.) which must be valid for a minimum of a year.

6. Employment verification letter/Contract: printed on company’s letterhead, confirming applicant’s occupation. Must include length of contract and compensation.

7. Original or certified copy of birth certificate. U.S. certificates require an Apostille Seal from the Secretary of State.

8. Medical Certificate: Letter signed by physician, stating overall health and plan of care, if applicable.

9. Criminal Background Check: issued by FBI. Requires an Apostille Seal from the Secretary of State.

10. Certification issued by the Ministry of Labor in the Dominican Republic, provided by employer.

11. Copy of diploma/certificate of highest level of education completed.

12. Copy of most recent resume.

13. Copies of 3 most recent consecutive bank statements.

14. $300.00 (non-refundable) application fee via money order, cashier’s check or company check made payable to Dominican Consulate in Chicago. Fee includes translations of the following: Birth Certificate, Medical Certificate & FBI background. If FBI background is more than 1 page (not including the Apostille Seal) please add $25.00 per page to the total amount.

15. Pre-paid self-addressed envelope: for passport/documents to be returned to applicant. We only accept USPS Priority Mail, USPS Express Mail, FedEx & UPS.

1.1.4.- Dependent Visa

1. Application form (click here).

2. Application letter: must include applicant’s name, address, citizenship, passport number, occupation and the reason why he/she is applying for a visa.

3. Original Passport- must be valid for a minimum of 18 months. Must have 2 blank visas pages available.

4. (2) Passport pictures.

5. Non-US Citizens must submit copy of immigration status (U.S. Green Card/U.S. Visa) which must be valid for 12 months.

6. Birth certificate.U.S. certificates require an Apostille seal from Secretary of State.Certificates not issued in the U.S. must have Apostille Seal/certification from corresponding Consulate.

7. Marriage Certificate, if applicable.U.S. certificates require Apostille seal from Secretary of State.Certificates issued outside of U.S. must have Apostille Seal/certification from corresponding Consulate.

8. Medical Certificate: Letter signed by physician, stating overall health and any known medical conditions, as well as treatment plan (when applicable).

9. FBI criminal background check with Apostille Seal from the US Department of State in Washington D.C.

10. Proof of income: Guarantor’s source of income; employment verification letter, etirement certification (SSI/401K) stating amount of retirement benefits, etc.Along with 3 consecutive bank statements to confirm income.

11. Application fee (non-refundable) of $300.00 via money order via money order/cashier’s check made payable to the Dominican Consulate. Fee includes translations of the following; birth/marriage/medical certificates and FBI. If FBI is multiple pages, include an additional $25 per page to the total fee.

12. Pre-paid self-addressed envelope for passport/documents to be returned to applicant. We only accept USPS Priority Mail, USPS Express Mail, FedEx & UPS.

This visa allows multiple entries to the Dominican Republic and is valid for 12 months.
Allow a minimum of 20-25 business days for processing. Incomplete applications will delay processing time.

1.1.5.- Tourist Visa

DESCRIPTION: Tourism purposes only.

As of 2009, Tourist Visas are no longer being issued in the United States.
Tourists must comply with the following;

U.S. Citizens
1. Valid U.S Passport. Must be valid for a minimum of 30 days upon arrival to the Dominican Republic.
2. Must purchase a tourist card upon arrival to the Dominican Republic.

NON-U.S. Citizens
1. Valid Passport from country of origin. Must be valid for a minimum of 30 days upon arrival to the Dominican Republic.
2. Valid(non-expired) immigration status in the U.S. (example: US Green Card/Permanent Residency, US Visa, US Travel Document, Advance Parole etc.) Must be valid for a minimum of 60 days upon arrival to the Dominican Republic.
3. Flight must originate from the United States.
4. Must purchase a tourist card upon arrival to the Dominican Republic.

**Please note that as of April 25, 2018 the tourist card is now included in the airline ticket as an additional tax fee. You are no longer required to purchase it upon arrival to the Dominican Republic. *

1.1.6.- Residence Visa

  2. Application letter: must include applicant’s name, address, citizenship, passport number, occupation and the reason why he/she is applying for a visa.
  3. Original Passport- must be valid for a minimum of 12 months. 2 blank visa pages required.
  4. (2) Passport photos.
  5. Non-US Citizens must submit proof of U.S. Immigration status (U.S. Green Card/U.S. Visa) which must be valid for a minimum of 12 months.
  6. Certified copy of Birth certificate. Requires an Apostille seal from Secretary of State (if born in the U.S.).
  7. Certified copy of Marriage Certificate, if applicable.Requires an Apostille seal from Secretary of State (if married in the US).
  8. Medical Certificate: Letter signed by physician, stating overall health.
  9. Copy of Dominican Passport/Dominican ID Card (cedula) of spouse/children (when applicable).
  10. Criminal Background Check: issued by the FBI.Requires an Apostille seal from the Secretary of State.
  11. Proof of income: Employment verification letter. Retirement benefit statement(SSI/401Ketc).Along with bank statements(3 consecutive months)to verify income. Retirement benefit statement require notary and Apostille Seal from Secretary of State.
  12. Warrantor letter signed by Dominican spouse, notarized and legalized by the Attorney General’s office in the Dominican Republic (when applicable).
  13. Proof of income of warrantor: Employment verification letter. Retirement benefit statements (SSI/401K) stating amount of retirement benefits. 3 most recent (consecutive) bank statements. Documents issued in the DR require legalization by Attorney General’s office. Retirement benefit statements require notary and Apostille seal from Secretary of State.
  14. Application& processing fee (non-refundable) of $550.00 via money order, cashier’s check or company check made payable to Dominican Consulate in Chicago. Fee includes translations.
  15. Pre-paid self-addressed envelope: for passport/documents to be returned to applicant. USPS Priority Mail, USPS Express Mail, FedEx and UPS.

This visa allows single entry to the Dominican Republic and valid for 60 days.
Allow a minimum of 20-25 business days for processing time. Incomplete applications will delay processing time. Additional supporting documents may be requested.

1.1.7.- Student Visa

DESCRIPTION: All students, who are not Dominican citizens, who wish to study in the Dominican Republic, must apply for a student visa.

This visa allows multiple entries to the Dominican Republic. Visa is valid for 12 months.

2. Application letter: must include applicant’s name, address, citizenship, passport number, occupation and the reason why he/she is applying for a visa.

3. Original Passport- must be valid for a minimum of 13 months. Please make sure that the passport is not full must have at least one (1) blank visa page.

4. (2) Passport photos.

5. Non-US Citizens must submit copy of immigration status (U.S. Green Card/U.S. Visa) which must be valid for 12 months plus an additional 30 days after exit date.

6. Letter of acceptance issued by the school that the applicant will attend.

7. Birth certificate. Requires an Apostille Seal from the Secretary of State.

8. Medical certificate: signed by physician, stating overall health and any medical conditions including treatment plans (if applicable).

9. Criminal Background Check: issued by the FBI. Requires an Apostille Seal from the Secretary of State.

10. Application fee (non-refundable) of $550.00, money order, cashier’s check or company check made payable to Dominican Consulate in Chicago. This payment will cover the application process and all translations.

11. Pre-paid self-addressed envelope: for passport/documents to be returned to applicant. We only accept USPS Priority Mail, USPS Express Mail, FedEx& UPS.

Must include (1) copy of every document submitted.
Allow 25-35 business days for processing. Incomplete applications will delay processing time.

1.1.8.- Missionary Visa

  2. Application letter: must include applicant’s name, address, citizenship, passport number, occupation and the reason why he/she is applying for a visa.
  3. Original Passport- must be valid for a minimum of 12 months. Must have 2 blank visas pages available.
  4. Non-US Citizens: Copy of US Green Card/US Travel Document
  5. Passport pictures.
  6. Birth certificate. US Certificates require Apostille Seal from the Secretary of State.
  7. Marriage certificate. US Certificates require Apostille Seal from the Secretary of State.
  8. FBI Criminal Background Check. Requires Apostille Seal from the Dept of State, Washington D.C.
  9. Guarantor letter from the religious institution in the Dominican Republic confirming the nature of the trip, length of stay and if any financial assistance will be provided. Letter must be legalized by the Attorney General’s Office in the Dominican Republic.
  10. Income verification: 3 consecutive check stubs and 3 consecutive bank statements.
  11. Application fee (non-refundable) of $300.00, money order, cashier’s check made payable to Dominican Consulate in Chicago. Fee includes translations of corresponding documents.
  12. Pre-paid self-addressed envelope for passport/documents to be returned to applicant. USPS
    Priority Mail, USPS Express Mail, FedEx & UPS.
